IOCongress 2018 Incident Reporting Form
To file a report with the IOCongress Ombudsperson, please fill out and submit this Incident Form. Prior to submission of this form, we recommend that you review the Congress Code of Conduct found here: http://www.iocongress2018.com/code-of-conduct. This form collects information regarding possible Code of Conduct violations. The completed form should contain sufficient detail to allow the Ombudsperson to begin an investigation, if warranted.
